By general contract agreement with transportation providers:
- Passenger fatalities, injuries, and major incidents must be reported to Veyo within one hour.
- All accidents must be reported to Veyo on the day they accident occurred.
- A written report must be submitted to Veyo within 3 working days.
Requirements may be more stringent based on client contract. All reports can be made using the following methods:

- Calling Trip Dispatch/provider support
Here is the information you will need to gather and report to Veyo:
- Provider name
- Provider contact name
- Provider contact information
- Passenger name
- Trip #
- Driver name
- Vehicle
- Date/time of pickup
- Date/time of accident
- Date/time of report to Veyo
- Location of accident
- Was anyone transported by an ambulance
- Accident Type (rear ended, hard break, t-bone, vehicle merged into each other)
- How many vehicles were involved
- What was the providers vehicle's motion right before the accident
- Details of accident
- Whether the passenger had a seatbelt on
- If a wheelchair passenger:
- Was the passenger seat-belated into the chair - describe the wheelchair securement process
- Was there a secondary lap (posey) belt in use
- Any applicable photos
- List of information of other parties involved in the accident
- Police Report Number (if applicable)
- Police jurisdiction where report filed (if applicable)
- Provider’s insurance company
- Claim number assigned by provider’s insurance company
- Detailed statement by the driver describing all events from the time of arrival at the pickup location
- Driver training records related to driving skills, passenger safety, wheelchair securement, etc.
